Fees & Billing

**Please note: I currently have limited availability for new patients at this time.**

To provide the highest level of care, I intentionally maintain a small caseload. This allows me to offer my patients dedicated time, flexible scheduling, and seamless coordination with other providers when needed.

My concierge services are offered on a fee-for-service basis, without a monthly retainer. For patients who require additional support between appointments — such as professional letters, care coordination, or review of outside records — I offer these services as needed (see Other Professional Services below).

If my current availability or fee structure doesn’t align with your needs, I encourage you to request a consultation at Choulet Performance Psychiatry^® with one of my trusted colleagues. Each clinician on my team shares my commitment to exceptional, personalized care.

Initial Consultations:
All new patient visits begin with a consultation. This appointment is an opportunity for us to determine whether we’re a good fit to work together. A treatment relationship is not guaranteed and will depend on clinical appropriateness and mutual agreement.

• Adult Consultation (60min) | $750
• Child & Adolescent Consultation (90min) | $850

Follow-Up Appointments:

• 30min Follow Up | $375
• 50min Follow Up | $625

Other Professional Services:
I make myself readily available to provide additional support outside of scheduled sessions. Any service requiring more than five minutes of time — including the following — is billed in 5-minute increments:

• Writing professional letters or documentation
• Reviewing or responding to secure messages, texts, or emails
• Phone consultations or coordination with family members
• Communication with outside providers
• Review of psychological testing or treatment records
• T​​​​​​​ime spent performing any other services on a patient's behalf

Insurance and Reimbursement:
Please note that I am a fee-for-service physician and do not contract with any insurance companies. However, I can provide a detailed superbill that you may submit to your insurance carrier for possible out-of-network reimbursement.
